Your Rights

Patients have the right to:

  • Access information about all SWHR (Southwestern Health Resources) care management programs and services.
  • Access to information regarding staff, credentials, and their role.
  • Be informed of their assigned case manager and the process for requesting a change if desired.
  • Receive support from care management in collaborating with their providers on care decisions.
  • Decline participation or disenroll at any time from a program or service
  • Be always treated with courtesy and respect.
  • Confidentiality, security, and privacy of sensitive information used by SWHR and its affiliates.
  • Access to the process for submitting a complaint.

Expectations of patients:

  • Follow the mutually agreed case management plan of care or inform the case manager if they cannot follow the plan of care.
  • Provide SWHR with the necessary information to ensure appropriate service delivery.
  • Inform their case manager and/or provider if they disenroll from the program.
