Patients have the right to:
- Access information about all SWHR (Southwestern Health Resources) care management programs and services.
- Access to information regarding staff, credentials, and their role.
- Be informed of their assigned case manager and the process for requesting a change if desired.
- Receive support from care management in collaborating with their providers on care decisions.
- Decline participation or disenroll at any time from a program or service
- Be always treated with courtesy and respect.
- Confidentiality, security, and privacy of sensitive information used by SWHR and its affiliates.
- Access to the process for submitting a complaint.
Expectations of patients:
- Follow the mutually agreed case management plan of care or inform the case manager if they cannot follow the plan of care.
- Provide SWHR with the necessary information to ensure appropriate service delivery.
- Inform their case manager and/or provider if they disenroll from the program.
